The concept of patient-centered care has been with us for decades. However, modern healthcare focusing on care in the doctor’s office and the hospital, where services and care providers reside, has created large gaps in patient care. For many reasons including shortage of personnel, our longer lives and cost of care, more of our health care is moving to our homes each year.
At the same time, we need to help patients become active and involved in their own health. When patients are facing multiple chronic conditions or serious illness, and in need of assistance, they and their caregivers play a key role in supporting their health care.
In today’s health care environment, the ability to collect meaningful data from patients at home is critical to understanding their health conditions and improving care outcomes.
