Based in California with a team located across the US, Innovatio.health is a healthcare data analytics and artificial intelligence group developing insights and solutions for healthcare in the home.

Our mission is to support the Continuous Intelligent Management™ of rising risk and complex patients, to improve the patient experience, as well as decrease avoidable ED visits and hospitalizations.

Our core product launching in 2024 is Carenauticaᵀᴹ, a health management app for patients and caregivers. Modular in design, it allows the user to select any or all of the interactive functions including medication tracking, wellness survey and messaging platform.

Our Story

Innovatio.health was initially founded to develop a microhospital and care navigation system centering care in communities and patient homes while connected to health systems. Technology enabled care would play a major role in providing the community centered care we envisioned.

Following further exploration and the completion of our first predictive algorithms for home health, we have determined real-time qualitative and quantitative data from patients and caregivers provide the best basis for building meaningful predictive models and delivering guidance.

Our core product launching in 2024 is Careanutica™, a health management app for patients with complex health conditions and their caregivers.

How Does Patient-Centered Care Help the Patient, the Caregiver and the Health System?

A patient with chronic conditions can both improve their conditions and prevent exacerbation by staying on top of their medications and following recommendations for meals and exercise. Some patients manage these issues on their own. Some need the support of caregivers.

When patients have a personal caring community whether with 3 or 30 friends/family, these relationships support the care of the patient. Patients are not alone.

Carenautica™ supports medication adherence and provides a daily window into the patient’s condition. If the patient is at the beginning of a decline in their condition(s), our AI-enabled app will alert friends and loved ones to communicate with the patient and determine if something is amiss or if mitigation is required.

Easy to understand information can guide the patient and their personal caring community to improve the patient’s situation, prevent exacerbation and potentially avoid hospitalization.

Avoiding hospitalization is best for the patient and best for the healthcare system.

Our healthcare system focusing on disease and specialty care is sorely lacking in preventive care support. Carenautica™ is designed to help the patient and their caring community to better manage health conditions at home and provide actionable information to their healthcare providers.
